Procurement of Application Form & Prospectus and counselling Fee
The Application Form (part-A & part-B), Bank Challan in triplicate and Prospectus can be
downloaded from the column “ENTRANCE NOTICE” of the website :

(i) Counselling Fee
Candidate willing to appear in PG Medical counselling (PGMAC)-2013 based on
Bihar State Rank List of NEET (PG)-2013 are required first to download the Bank
Challan in triplicate and using the same are required to deposit the counselling fee,
` 300.00 (Three hundred Rupees) in case they are of General (Unreserved) /BC / EBC
category and ` 225.00 (Two hundred twenty five Rupees) in case they are of SC/ST
categories.
After depositing the required amount of counselling fee through the Bank Challan,
each candidate will get transaction ID / Journal no. as mentioned on the paid challan.
(ii) Part A and Part B of the Application Form
Thereafter the candidate is required to fill in Part-A of the Application online and
download the complete Application Form (filled up and submitted part-A of the
Application Form and blank part-B of the Application Form). The part-B of the
Application Form is to be filled in manually.
Passport size photograph of the candidate, identical to one as used for preparing
Admit Card of NEET (PG)-2013 is to be pasted at the specified space of Part-A of the
Application Form. The candidate is required to put his / her full signature in English and
Hindi at the specified places of Part-A & Part-B of the Application Form.
(iii) Enclosures
Self attested photo copies of the following documents as required are to be enclosed
with the completed Application Form. i. Board’s Copy of the Challan as proof of paying the required counselling fee.
ii. Residential Certificate.
iii. Caste Certificate.
iv. College leaving certificate.
v. Rotatory Internship Completion Certificate
vi. MBBS passing certificate
vii. Mark sheets of MBBS (I, II & III Parts).
viii. No objection certificate from employer.
ix. Medical Registration Certificate.
x. Admit card of PG Medical Test i.e. NEET (PG)-2013
xi. Three photographs identical to one used for preparing the Admit card of NEET
(PG)-2013.
xii. Any other certificates in support of eligibility as required and mentioned in the
prospectus of PGMAC-2013 (available on website).
(iv) Submission of the Application Form
The completed Application Form (Part-A filled in ‘Online’ and Part-B filled in
manually) alongwith the required enclosures is to be submitted by hand personally at the
time of counselling for due verification of eligibility and identity against the original
certificates / documents as presented by the candidate at the time of counselling.
After due checking / verification and establishing the eligibility of the candidate
appearing for the counselling, the Course / Institution will be allotted based on merit-cum-option
